Minnesota School Immunization Requirements for Students
Under Minnesota law (Statute 121A.15 and Rule 4604.0400), all children enrolled in school from birth through 18 years of age must either:
- Be immunized against certain preventable diseases, or
- File a valid medical or conscientious exemption
At New Prague Area Schools, each student must submit documentation before the first day of school showing either:
- Proof of receiving the required immunizations, or
- A completed exemption form
As the end of summer approaches, this is an ideal time to schedule a physical for your child to ensure that all required immunizations are current and compliant with school requirements.
